Criteria (source: Kepdirjen Yankes No. HK.02.02/D/43961/2024, Bab II huruf B (Kriteria Surveior))
- Indonesian citizen
- At least 35 years old
- Physically healthy
- Good communication skills
- Never involved in a criminal act (final court ruling)
- Medical/dental specialist; physician/dentist with a relevant health master's degree; or registered nurse/relevant health master's degree (other health workers with a profession/master's degree when needed)
- Experience working in a hospital
Survey scope:
- Hospitals
Files (PDF/JPG/PNG, max. 5 MB each):
- Curriculum vitae (CV)
- STR (registration certificate)
- Latest diploma
- Proof of work experience
- Medical certificate from a physician
- Statement of no criminal conviction
